The University Clinic for Diabetology, Endocrinology, Nutritional Medicine and Metabolism (UDEM) cares for patients with complex metabolic diseases at the highest university level. To strengthen our interprofessional team, we are looking for a committed and motivated personality as a Diabetes Counselor HFP/DAS/HöFa1 What awaits you Counseling and training of outpatient and inpatient patients with diabetes mellitus (Type 1, Type 2 and gestational diabetes) Support and instruction in insulin therapies as well as modern diabetes technologies (CGM, insulin pumps and automated insulin delivery systems) Promotion of self-management and health literacy of patients and relatives Conducting individual and group training sessions Interprofessional collaboration with doctors, nutritionists, nursing professionals and other specialist disciplines Documentation in the electronic patient dossier Participation in the further development of diabetes counseling What you bring with you Diploma as a nursing professional HF/FH or an equivalently recognized training Completed further training as a Diabetes Counselor HFP, DAS or comparable qualification (Master's degree) Experience in the care of persons with diabetes, ideally in an acute hospital or outpatient setting Enjoyment of patient-centered, evidence-based counseling High social, communication and counseling competence Independent, structured and responsible way of working Interest in innovation, digitalization and the further development of modern diabetes care Our offer Responsible and varied activity in a university center Committed, interprofessional team with high professional competence Collaboration in an innovative environment with most modern diabetes technologies Attractive continuing education and further training opportunities Contemporary employment conditions Questions about the position?
