Health Specialist as Spitex Coordinator (60-100%)

On direct commission from our client, a sympathetic, renowned and strongly growing Spitex organization (PA model) with a sympathetic company culture (headquartered in Stadt Bern and Zürich, central location, parking spaces in front of the house), we are looking for an experienced health specialist for the coordination and case management for the expansion of the team at the new location in Bern. Health Specialist as Spitex Coordinator (60-100%) In this versatile function, you are part of a collegial, dynamic team (Dipl. Nursing Specialist/FaGe/PA in care) and, as a competent specialist, take over the accompaniment/coaching of the nursing relatives (PA) who take over the care/nursing of the clients on site. You work in case management, are on site for needs assessments, and support the PA with regular visits to clients and in their training/instruction. You support the case documentation, coordinate billing with the health insurance companies, and ensure optimal client care and provision of care services. As a communicative person, you also find it easy to work together actively and exchange information with internal offices (deployment planning, Dipl. Nursing colleagues) and external contacts (authorities, health insurance companies, caring relatives, and care team). To be successful in this exciting job, you have completed training as a health specialist (FaGe) or equivalent education and/or further training (or are in training). You have also gained versatile practical experience in coordination in the care sector (ideally including Spitex). You enjoy supporting different people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with high independence, are well organized, and have a great ability to get involved, as well as a healthy dose of humor.
