Your mission
As a reference nurse, you ensure the role of situation manager. You perform and delegate quality services in order to guarantee the health, safety and well-being of clients at home, by identifying their social issues. At their place of living, you define with them the most adapted solutions to their situation, thus developing a privileged relationship of trust.

Your responsibilities
Ensure the responsibility of the reference role with clients presenting disorders relating to mental health and/or psychogeriatrics
Evaluate and implement services according to the needs of the client and their surroundings and provide the services planned in the intervention plan, by conducting evaluation, orientation, advice interviews as well as support in administrative and financial procedures
Ensure medico-delegated care services, hygiene and comfort as well as support for our clients
Accompany auxiliary collaborators in order to guarantee the quality of services
Create and maintain lasting links with partners of the regional medico-social network (including volunteers and user groups)
Mobilize the multidisciplinary skills of the CMS care team, while sensitizing them to the problems posed by psychiatric or psycho-geriatric care and proposing prevention measures
Document the client's computerized file according to institutional directives to allow continuity of care
Your profile and your skills:
Bachelor in nursing HES or equivalent title (recognition of foreign title by the Swiss Red Cross), with specialization in mental health
Minimum 6 months of experience in psychiatric care
Good knowledge of the Swiss socio-sanitary field (first job in Switzerland)
Capacity for analysis and synthesis
Good mastery of the French language with solid writing skills
Capacity for organization, priority management and taking initiative
Good knowledge of usual IT tools
Capacity to communicate and negotiate
Capacity to work in a multidisciplinary team.

Schedules mainly during the day (7h30–17h00), with only one evening worked every two weeks until 22h00 and one weekend worked every eight weeks.
