Pflegefachperson HF / FH (40-100%) Your versatile tasks are: • You are active in direct care and, in complex care situations, take on the consultation and instruction of customers, relatives, and employees • You perform needs assessments using the InterRAI instrument *(RegioSpitex Limmattal will cover the costs for the InterRAI further training)* • As a case manager, you are responsible for the planning and implementation of the nursing process for your customers • You play an active role in cooperating with the supporting, interdisciplinary network and coordinate the corresponding services for your customers You bring with you: • You have a degree as a Dipl.
