Rigorous monitoring: Frame and re-evaluate regularly the situations, coordinate the essential actions for safety and continuity of care. Documentary management: Responsible for the management of OPAS files and the updating of client files in accordance with the directives. Interdisciplinary collaboration: Work closely with the entire network (CMS team, proches-aidants, doctors, partners) to ensure optimal care. YOUR PROFILE You hold a diploma of infirmier-ère HES or an equivalent title, reinforced by significant experience of 2 years, preferably in home care. You are polyvalent-e, listening and autonomous; possess excellent relational skills, negotiation and show a pronounced interest in home maintenance. Your sense of organization, your ability to anticipate needs and adapt to any type of situation and your competence in stress management are essential qualities.
