Specialist for Age-Related Issues (60%, m/f/d) The Altersversorgungsregion Rheintal is looking for the management of the Birsfelder Fachstelle für Altersfragen as of 1. October 2026 or by arrangement a Specialist for Age-Related Issues (60%, m/f/d) The Altersversorgungsregion Rheintal consists of the municipalities Augst, Birsfelden, Giebenach, Muttenz and Pratteln and is organized as a special-purpose association. The service region informs and advises older people as well as their relatives who are dependent on support or are considering entry into an old age and nursing home. In Birsfelden, Muttenz and Pratteln, these tasks are fulfilled by the respective Fachstellen für Altersfragen. Your main tasks are: Advising and accompanying older persons and their relatives and family members Mediation and coordination of suitable offers as well as cooperation with service providers and other providers such as Spitex, old age and nursing homes and other specialist organizations for aging Ensuring advice and needs assessment before an initial entry into an inpatient care facility Interdisciplinary cooperation, collaboration and networking work between institutions, internal and external specialist groups and committees Cooperation with service providers for outpatient, intermediate or inpatient offers for mediation after a needs assessment Organization and coordination of prevention offers regarding age and health Responsibility for all administrative work related to the tasks (such as case management, maintaining statistics and other correspondence) You have a completed degree in Social Work FH/HFS, in the nursing field HFS or a comparable training. Existing professional experience in social work as well as with older people in the field of age / geriatrics is an advantage. Knowledge of social insurance and/or adult protection law as well as specialist knowledge of age-specific diseases are also an advantage.
