Pro Senectute Kanton Zürich has been committed to the welfare of older people as a private, non-profit foundation for over 100 years. We are the most significant specialist organization for all questions surrounding old age. With diverse services and advisory offers, we provide competent support to seniors, their relatives, as well as authorities and institutions throughout the canton. For our Dienstleistungscenter Limmattal and Knonaueramt, due to the retirement of the current job holder, we are looking for, as of 1 November 2026 or by arrangement, a Coordinator CareNet+, 60-80 % ## CareNet+ is a coordination offering for complex care situations. The offering is aimed at people aged 60 and over in the Bezirk Affoltern, their relatives, as well as involved specialist offices and institutions. CareNet+ ensures the coordinated, case-related cooperation of all service providers in the health and social sectors and thus contributes to coordinated and effective support. Your tasks ### • You bear the overall operational responsibility for the CareNet+ offering and promote interdisciplinary cooperation in the district. • You advise and accompany demanding case constellations and ensure coordinated, needs-based support. Your interventions take place promptly and, if necessary, on-site. • You take over case management and control according to case management principles and are responsible for the documentation in the case management system GERAS. • You identify and access new cases independently and actively develop the offering further. Your profile ### • You have completed training in the social sector (FH) or an equivalent tertiary degree. • You bring several years of professional experience; experience in Case Management is desirable. • You think in networks, work in a structured and independent manner, and maintain an overview in complex situations. In doing so, you are characterized by a high level of customer orientation. • As a responsible and communicative personality, you value exchange with people and act in a reflective and solution-oriented manner. • With your advisory competence and winning manner, you succeed in convincing different stakeholder groups and developing viable solutions together.
