Your main missions will include : • Providing home nursing care according to medical prescriptions and the specific needs of patients. • Ensuring technical care such as dressings, injections, blood tests, clinical monitoring, and other nursing acts. • Managing and monitoring medication treatments to guarantee their proper administration and effectiveness. • Assessing the health status of patients, detecting clinical changes, and adapting care accordingly. • Supporting patients in maintaining their autonomy and promoting their quality of life at home. • Ensuring clear and benevolent communication with patients, their loved ones, and healthcare network stakeholders. • Participating in the therapeutic education of patients and their entourage to help them better manage their health on a daily basis. • Transmitting necessary information for patient follow-up and contributing to the coordination of care with all partners. \------- Profile sought: This position requires great autonomy, excellent organizational skills, and the ability to make decisions adapted to the different situations encountered at patients' homes.
