This is what you can expect <br /> • Carrying out admission assessments at referring institutions to assess the rehabilitation capability of registered patients • Coordination and agreement within the interprofessional treatment team to assess admission situations and ensure a needs-based patient admission • Participation in the optimization of processes at the interfaces between referrers, admission planning and treatment teams • Taking over the nursing day-to-day coordination and supporting the management of the day-to-day business • Taking over occasional nursing activities and direct patient care as needed <br /> This is what you bring <br /> • Completed training as a Dipl.
