Spitex Coordinator / Registered Nurse HF/FH (60-100%)

On behalf of our client, a sympathetic, renowned and strongly growing Spitex organization (PA-model) with a sympathetic company culture (headquarters in, among others, the cities of Bern and Zurich, central location, parking spaces in front of the building), we are looking for an experienced Dipl. Registered Nurse HF/FH for the coordination of the team and case management to expand the team at the new location in Bern. Spitex Coordinator / Registered Nurse HF/FH (60-100%) # In this versatile role, you are part of a collegial, dynamic team and, as a competent specialist, you take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications and deployment planning for the FaGes and caring relatives (PA), who care for the patients on site. You take over case management and coordination with the health insurance companies, provide advisory support to the teams and assist in advising clients (by telephone, occasionally also on site - e.g., during status assessments). Together with your team, you ensure optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with a wide variety of contact persons (management, FaGes, care team, external bodies such as health insurance companies or authorities) easy. Registered Nurse HF/FH or an equivalent further education (or are currently in training for it). In addition, you have already been able to gain versatile practical experience in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well-organized and possess great capacity for introduction as well as a healthy dose of humor.
